Requirements- In Spa Service
A consultation is required minimum 7 days prior to appointment
A consent release from your doctor will be needed
Areola- Minimum 6 Months after surgery

Insurance
If you are coming directly to the Spa for your procedure, please ensure to bring your insurance card to your consultation, so we may contact your insurance prior to see if they need pre-approvals etc.
Please note at this time, all claims to Blue Cross Blue Shield are on hold if coming directly to us for your procedure and we ask that you contact them directly before the procedure. You will have to pay-out of pocket and file a claim after the procedure. But you must call before to get pre-approvals.
